Peartree Medical CentreSix patients attended on 4th July to discuss their wider health and well being needs with the Care Co-ordinator and Interpreter. Assistance was offered on understanding benefits and access to funding for accommodation adaptations.
The Care Co-ordinator is always available to assist patients with accessing this type of support or information and patients should arrange this through reception.
We will hold another Signpost event later in the year. Text messages will be sent out to all patients over 16 years of age with a current mobile number registered with us.
